I just need to make sure you're aware of a couple things before proceeding:

FIRST

Have you checked my TRAVEL SCHEDULE to see if I'm in your area on the day you wish to see me? If not, please click the buton on the left at the bottom of this page.

SECOND AND MOST IMPORTANTLY

Although I ask for a lot if information on my APPOINTMENT REQUEST form, it is not necessary to fill out the form if you are uncomfortable doing so....just shoot me an email with the information I need...(I respect that fact that some people do no like filling out forms).

Before you proceed please check the following to make sure you have all the necessary information I need to get you screened prior to any appointment.

I NEED:

PROVIDER INFORMAITON

Two (2) Provider References or One (1) Provider Reference and your Employment information. This must include the following:

1. Provider's Name

2. Provider's Phone Number

3. Provider's Email Address

4. Provider's Website Link or link to her Eros web page (not just Eros.com)

NOTE: Your Provider references MUST be Independent Provider's (not affiliated with an agency). Her website must show some nudity (at least topless)

NOTE: Your Provider Reference's MUST also have a history of reviews on TER or Big Doggie (at least 20 reviews each).

Please be sure to contact your Provider reference(s) and let them know I will be contacting them for a reference on you...some Providers do not give out client references unless the client asks them to do so first.

EMPLOYMENT INFORMATION:

1. Name of company you work for

2. Company address

3. Link to Company's website

4. Company telephone number as listed with Directory Assistance....this is the phone number I would be

given if I dialed 411 and asked the operator for the number to your company.

PLEASE NOTE: Your Employment Information (this is only used as a final source of verification, should I not be able to reach your 2 Provider References.)

If you can not provide all of this information please send me what you can and I will take a look at it and go from there...no promises though...but hopefully we can work out a level of security that we are both comfortable with.
